- Fill out and submit the PLAA MEDICAL RELEASE FORM online.
- PAY ONLINE
$750 for Morning Arts & Music Program (9am-noon)
$750 for Afternoon theater program (12:30pm-3pm)
($250 deposit per program)
Please note: Your space is not reserved until both forms are submitted and deposit has been received
